Long-term Care

This team helps people aged 18 and over who have a long term illness or disability to carry on living independently in their own homes. Each person will have a care co-ordinator who makes sure they receive the services that they need. The team also supports people who are nearing the end of their lives, and supports families and friends who care for someone who has a disability or a long-term health condition.

Staff in the Long Term Support Team include district nurses, an occupational therapist, social workers and a nurse from the St David’s Foundation.
